This is the second episode in my three-part series on reversing atherosclerosis, and it tackles the question I get asked more than almost any other: outside of pharmaceuticals, can a supplement actually reverse the plaque that has already built up in your arteries? Not simply slow it down, but genuinely shrink it. It is also personal for me. In 2024 a CT angiogram showed I had a small amount of soft plaque in my left anterior descending artery, so I went looking for honest answers in the human trial data.
The supplement aisle is full of bold promises, and social media is full of confident claims, but the evidence is far more uneven than the marketing suggests. In this episode I work through eight supplements and rank them by the strength of the human evidence, separating what genuinely moves plaque from what only moves a biomarker. Because as you will hear, improving a number on a blood test is not the same thing as improving the health of your arteries.
What we cover:
- Why omega-3s, and specifically pure EPA, have the strongest evidence for regressing plaque, and what the EVAPORATE, CHERRY, JELIS, and REDUCE-IT trials actually showed
- The heated debate between REDUCE-IT and STRENGTH, the mineral oil controversy, and why the form of omega-3 you take may matter more than the dose
- Aged garlic extract, and the imaging studies suggesting it may slow plaque progression
- Vitamin K2, the calcification paradox, and why directing calcium away from your arteries is more complicated than it sounds
- Niacin as a cautionary tale: how a supplement can raise your “good” cholesterol and still do nothing for your risk
- Where CoQ10, berberine, nattokinase, and pomegranate extract really sit once you look past the headlines
- The David Sinclair nattokinase claim, and what the data actually says
- My own plan after that scan, and a preview of part three on the pharmaceuticals
My honest takeaway is that only one of these eight has strong evidence behind it, and even then it is a prescription, not a tub from the supplement aisle. The rest range from promising but early to genuinely cautionary. Supplements can be useful additions, but they are never a replacement for the proven basics, so know your numbers and work with your doctor.
